NOTICE: Transcripts will only be forwarded to other institutions for students who have attended a minimum of one full quarter, are in good standing with the accounting department, and have completed a minimum of 180 lessons.

You are encouraged to speak with an admissions counselor for a more detailed evaluation of your particular needs at no cost. You may also choose to complete placement testing (usually requires 6-12 weeks) and portfolio evaluation for a personalized graduation plan and expected graduation date. The cost is the same as attending for one quarter and requires that you enroll as a student with the intent to transfer at the end of the quarter if you find you are unable to complete your recommended plan.

NOTE: You may want to open your word processing software program and type up the answers to these questions at your leisure then cut and paste your answers when you are ready to submit this form.
